How we obtain and store your information
4Cyte pathology obtains information from request forms and other documents sent in with pathology requests, by phone or by fax. The information is updated when we recieve documents that supersede previous information, or by phone when the any details change.
The purpose and intention of 4Cyte storing your information
1. Patient care:
The primary purpose of the all information kept by 4Cyte Pathology is to provide optimum care for patients and to provide support to other health care providers. This includes, but is not limited to, reporting pathology results with identifying information to treating health care providers.
2. Providing our services and billing
Patient personal information is used to identify samples for processing. This enables 4Cyte Pathology to accurately report pathology results to the correct health care provider for each patient.
We also use patients personal information to ensure all reference ranges and billing rates are calculated and set correctly.
3. Training and quality assurance
4Cyte Pathology may use pathology reports internally for training and quality assurance purposes.
4. Other Disclosures
We may be legally obliged to release reports from time to time without notice.
When we do not have all of your required personal information
A lack of information may impair our ability to process samples or report results. Sampled that are not labeled with the correct identifying information may need to be recollected.
Distribution of your personal information from 4Cyte
4Cyte may supply any information we have when we are either clinically or legally obliged to do so. This includes reporting results to any healthcare provider listed on a request form, any healthcare provider who is treating you or any legal matters where our records are subpoenad to court.
No information will be released where there is no legal or clinical obligation to do so without the consent of the patient named on the form or the carer for the patient or when the parent of the patient if the patient is younger than 16 years old.
Accessing your own personal information
You are entitled to any information 4Cyte have stored with regard to your personal records, unless otherwise specified. To obtain a copy of your results please send a written and signed request to:

Your results will then be issued to you by post to the address listed on your request form. There is no charge for accessing your results, but there may be a fee associated with collating your records.
